The Church of Christ is organized, probably at the Smith home in Manchester, New York. The six organizing members are probably Joseph, Joseph Sr., Hyrum, and Samuel H. Smith; Oliver Cowdery and Joseph Knight , though Orrin Porter Rockwell, Sarah Rockwell, Lucy Smith, and Martin Harris have also appeared on some lists of six.

Joseph Smith Jr. and five others, in front of 56 people in attendance, formally organizes The Church of Jesus Christ in Fayette, New York. minutes Joseph Smith Jr. is ordained an elder, as is Oliver Cowdery. Joseph Smith Jr. is appointed as the Prophet, Seer, and Revelator for the entire church. (3)
[Joseph Smith] The Church of Christ is organized in New York state with offices of elder, priest, teacher, and unordained apostles. Aside from Joseph Smith's ordination as "a prophet," previous baptisms and ordinations are re-performed. Early sources, including the 1833 Book of Commandments, say the church was organized in Manchester (near Palmyra), rather than in Fayette, New York, as traditionally claimed. (4)
[Joseph Smith] The Church of Christ is organized; six revelations are received (BC 17-22; LDS D&C 21, 23; RLDS D&C 19, 21). Cowdery is ordained an elder. Joseph Jr. is ordained an elder, also prophet and seer by Cowdery. Joseph Sr., Lucy, Harris, and Sarah Rockwell are baptized in Crooked Brook. (5)
[Joseph Smith] Formally organizes the Church at the home of Peter Whitmer Sr. in Fayette (see D&C 20:1). At first the official name of the Church is the Church of Christ. On May 3, 1834, the name is changed to The Church of the Latter Day Saints. On April 26, 1838, the name is changed by revelation to The Church of Jesus Christ of Latter-day Saints (see D&C 115:4). Oliver Cowdery preaches publicly for the first time as an official representative of the newly formed church. In Seneca Lake he baptizes Hiram Page, Catherine Whitmer Page, Christian Whitmer, Anne Schott Whitmer (Christian's wife), Jacob Whitmer, Elizabeth Ann Schott Whitmer (Jacob's wife), and Mary Page.
